Supervisory Committee Form

First consult the  Physics PhD Program Information, Policies, and Procedures pdf document and also the General Exam Information pdf document.

The information you enter into the form below forwards automatically as an e-mail to the Graduate Program Coordinator, who will check on MyGrad to see that you have completed all required courses and also reviews the make-up of your proposed committee. The GPC will contact you by e-mail with approval or with some alternate suggestions or additional questions.

When approved, the Graduate Program Adviser establishes your committee with the Graduate School.
You and the members of your committee will receive a confirmation email from the Graduate School.
