Exam accommodations for students through Disability Resources
It is the policy and practice of the University of Washington to create inclusive and accessible learning environments consistent with federal and state law.
If you have a temporary health condition or permanent disability that requires accommodations (conditions include but are not limited to: mental health, attention-related, learning, vision, hearing, physical), please contact Disability Resources for Students (DRS) at 206-543-8924 or firstname.lastname@example.org or http://depts.washington.edu/uwdrs/ as they can provide resources and coordinate establishment of resonable accommodations.
Once you have met with the Disabilities Resources for Students counselors and have exam accommodations, your professor should receive an email with your testing accommodation requests, along with a link to fill out a testing contract. After the testing contract has been filled out (by either your professor or by the program coordinator), you can schedule your exams for the quarter.
For students in Physics honors 12x course: All exams will be taken at the DRS testing center. You will need to schedule your midterm exam at least FIVE days in advance in order to take it at the DRS testing center. You must schedule your final exam at least TEN business days in advance in order to guarantee a spot at the DRS testing center.
For students in Physics 114, 115, 116, 121, 122, or 123: Midterm exams will be taken in the Physics building, unless you have an accommodation that cannot be met in the Physics building. TAs will proctor the exams and have a list of your testing accommodations. The final exam will need to be taken at the DRS testing center. You must schedule your final exam at least TEN business days in advance in order to guarantee a spot at the DRS testing center.
